Print and sign the completed form.
Alternatively, you can print the form and complete it by hand. Please write clearly to avoid delays in processing your application.

How to submit your application
Step 1: Pay the fee
Pay the application fee at an NZ Post Shop that offers the 'pay a bill' service. You cannot pay the application fee at a Post Centre.
Step 2: Submit your form and supporting documents
You can submit your application form, supporting documents and related receipts:
